چکیده

The retrograde atrial activation sequence constitutes an initial important clue to elucidate the tachycardia mechanism during diagnostic electrophysiological testing in patients with supraventricular tachycardia. However, in some cases its correct analysis is challenging.

Letter by Mahajan Regarding Article, "A Narrow QRS Complex Tachycardia With Apparently Concentric Retrograde Atrial Activation Sequence"

Arias et al in the article 'A Narrow QRS Complex Tachycardia with an Apparently Concentric Retrograde Atrial Activation Sequence' describe a case with spontaneous intra atrial block along the mitral isthmus to explain the change in atrial activation [1]. This phenomenon has been described during radiofrequency ablation while ablating along the lateral mitral annulus for a left free wall pathway...

Narrow QRS Tachycardia with Alternate Wide QRS Beats: What is the Mechanism?

A 44-year old lady underwent electrophysiology study for recurrent palpitations and documented narrow QRS regular tachycardia. The baseline ECG showed subtle preexcitation that was easily manifest on atrial pacing. The retrograde atrial activation sequence during ventricular pacing was eccentric suggesting retrograde conduction over the accessory pathway. A regular narrow QRS tachycardia with c...

Electrocardiographical case. Narrow complex tachycardia provoked by the Valsalva manoeuvre.

A 19-year-old Chinese man presented with tachyarrhythmia during a human centrifuge session while performing a Valsalva manoeuvre at near normal (+1.4 Gz) centripetal forces. It was likely that the hydrostatic effects of sustained centrifugal forces, the distortion of the heart's shape, as well as swings in the autonomic dominance pre- and post-Valsalva manoeuvre, contributed to the tachyarrhyth...

Letter by Namboodiri Regarding Article, "A Narrow QRS Complex Tachycardia With Apparently Concentric Retrograde Atrial Activation Sequence" - Alternative Mechanisms

Read with interest the article by Arias et al on a case of orthodromic atrioventricular reentry (ORT) mediated through the left free wall (LFW) accessory pathway(AP) with an apparently concentric atrial activation sequence [1]. Based on the observation that the atrial signals during the tachycardia were earlier in the His bundle (HB, anterosuperior septum) compared to the proximal coronary sinu...

Participation of fast and slow A-V nodal pathways in tachycardias complicating the Wolff-Parkinson-White syndrome. Report of a case.

Electrophysiological studies in one patient with type B pre-excitation and dual A-V nodal pathway revealed several types of paroxysmal narrow QRS tachycardia (PSVT). One type of PSVT reflected antegrade fast A-V nodal pathway and retrograde anomalous pathway conduction. This PSVT was characterized by early retorgrade activation of right atrial appendage, P following QRS and cycle length of 290 ...
